Declaration must be made on or before the entry deadline, and such declaration will apply to the Zone Championships, and (if advancing) to subsequent Provincial and National Championship rounds.

I hereby grant permission to any of the affiliated local, provincial and national bowling associations and Bowl Alberta and its affiliates, the right to collect, use and disclose, at their discretion any information about me and my participation in the above event (not limited to information contained in this registration package) for publicity, advertising or other promotion of the event or for the purpose of acknowledging or publicizing my achievement at that event. I understand that this may include written, pictorial or video materials.
